All summer campers must have the permission form filled out and returned to the school prior to summer camp beginning. In addition, for the safety of our campers we require a school t-shirt to be purchased. Download the t-shirt form, fill it out and return it to the school with your check made out to Ivy Hall Day School. You can also fax the forms back to the appropriate school (see our home page for fax numbers).
